BMJ Case Rep Published online: [please include Day Month Year] doi:10.1136/ bcr-2013-009759 DESCRIPTION A 22-year-old man with a history of intravenous heroin misuse, presented with 1 week of fatigue and fever. Blood cultures were positive for methicillin-sensitive Staphylococcus aureus. Physical examination showed multiple painful 1– 2 mm macular rashes on the palm and soles bilaterally (figures 1 and 2). Splinter haemorrhages (figure 3) and conjunctival petechiae (figure 4) were also noted. A transoesophageal echocardiogram demonstrated a 16-mm vegetation on the mitral valve (figure 5). Vegitations >10 mm in diameter and infection involving the mitral valve are independently associated with an increased risk of embolisation. However, he decided medical management after extensive discussion and was treated with intravenous nafcillin for 6 weeks. He returned 8 weeks later with acute shortness of breath and evidence of a perforated mitral valve for which he subsequently underwent a successful mitral valve repair with an uneventful recovery.

Dermatologic manifestations of infective endocarditis*

Despite advances in diagnosis and treatment, infective endocarditis still shows considerable morbidity and mortality rates. The dermatological examination in patients with suspected infective endocarditis may prove very useful, as it might reveal suggestive abnormalities of this disease, such as Osler's nodes and Janeway lesions. Osler's nodes on the dorsum of the foot.

Osler's nodes developed on the dorsum of the left foot of a patient with enterococcal endocarditis and on the dorsum of the right foot of another patient with Staphylococcus aureus endocarditis. The lesions were erythematous, raised, and very painful, but they resolved completely. There was no evidence of other embolic phenomena at the time the Osler's node appeared.

Janeway lesions: a valuable clinical sign in patients with infective endocarditis

E mbolic events are known clinical stigmata of infective endocarditis occurring in 20 to 50% of cases and are often associated with protracted bacteremia. Janeway lesions are an example of this embolic phenomenon and can be a silent clinical sign (1). In cases where patients are incapacitated to a point where history acquisition becomes challenging, identification of Janeway lesions can be an i...
